Hello my dear Friend
MAT is conducted by the AIME Four Times in a Year in the Month of the Feb, May, Sep , Dec
So you will have a Lot of chance and that to There is No Restriction about the Number of Attempts
That is the reason why you can apply for MAT in number of times

But there is No Requirement of Percentage Cut off
There is No Age limit as well
Coming to How to apply ---- you can go to Open the "Official website of AIME "

There you will find a "Online Application Form " before you will try to fill the form you can Pay the Amount by means of DD , with Credit or Debit card
Then it will ask about the details of the DD in Application form
